IACH for Pharmacists
Proper medication management is an essential part of the care of patients with hematological diseases. An aging population, an increase in anticancer treatment options, and high rates of comorbid conditions make navigating general medication reconciliation complicated. There is a rapidly growing body of literature highlighting the key role of clinical pharmacists when it comes to polypharmacy, medication assessments, necessary adjustments regarding drug-drug interactions, management of side effects, as well as education of other team members etc.
Overall, clinical pharmacist interventions have the potential to significantly improve patient care.With this background, the IACH will soon be launching a new educational activity dedicated to pharmacists, entitled “The IACH for Pharmacists”.
